Many thanks for booking an appointment with Mr Yazan Masannat, We will contact you to confirm the appointment details. You are expected to spend an1-2 hours in the clinic as you will be seen by Mr Masannat for the assessment and then sent to have the needed scans +/- biopsies and seen after for the results. Mr Masannat is approved by most private medical insurers, listed below is some of commonly used ones Aviva, Aetna, Allianz, Axa PPP, Bupa Claims & Bupa Global, Cigna & Cigna International, Exeter Family Friendly, Healix, Vitality Health, WPA If you are using private medical insurance please ensure that you obtain authorisation for all consultations and treatment in advance otherwise you will be personally liable for costs not covered by your insurer. PLEASE SEE OVER FOR IMPORTANT INFORMATION CONCERNING YOUR APPOINTMENT Outpatient Appointment Thank you for choosing to see me at for your outpatient consultation. This letter sets out some important information that I am required by law to provide to you. This is for your information only and is not a bill. As this includes information about my charges, if you do not have private medical insurance but someone else will be paying your bill, you may wish to pass a copy of this letter to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charges which they do not pay. Consultation Fee My fee for an initial consultation won't exceed £300.00 and the follow-up consultation fees won't exceed £220.00. Following your consultation you may need certain tests (such as blood tests or imaging, for example Mammogram, ultrasound scan, MRI or CT scan) to help me diagnose your condition. The fees for those tests will be determined by the hospital and charged to you, or your private medical insurer, separately. If there are any additional fees I will let you know what those will be. Financial Interests I am legally obliged to tell you if I have any financial interests in the hospital or any equipment. I can confirm I do not have any such interests.
